• Facilities Management: Housing Work Order Request Form

  • DO NOT SUBMIT ONLINE FORMS FOR EMERGENCIES. 

    Welcome to the form for Housing work orders.

    Our office hours are Monday-Friday, 8:00am-5:00pm. If your request is urgent or an emergency, please contact us directly via phone at (310) 338-7779 during our operational hours. 

    Outside of our business hours, including weekends, please contact the Department of Campus Safety Services (CSS) at 310-338-2893 and they will be able to dispatch our after hours technicians to assist. 

  • How to fill out form:

    Make sure to take the time to note all your facilities issues on one form.

    The dropdown menu lists your options and a comment box is provided for brief description of the issue. Make sure to add a detailed location so we can attend to the issue efficiently. 

    The table below summarizes examples for each option, along with some additional information.

    Custodial Stains/ carpet cleaning, sanitary issues, clean up in public spaces. 
    Electrical Lights out, outlets not working, no power to appliances, outlet covers missing/broken, smoke detector
    Lockshop Diplock door batteries. Door handle, Dead bolt issues.
    Pest Control An additional housing form is needed which will be available here. 
    Plumbing Toilet seat broken/falling, Clogged drains, showers, leaks.
    Screens/Blinds Missing Screens, broken cord, blinds not functioning properly
    Furniture University provided tables, chairs, couches, beds (see below for configurations)
    Washer/Dryer DO NOT SUBMIT WORK ORDER. For service/repair, report 6-digit machine ID to 1.800.342.5932 or fixlaundry.com
    Elevator DO NOT SUBMIT WORK ORDER. Call us at 310.338.7779 or Public Safety after hours at 310.338.2893
    Potential Organic Growth Any concerns related to an occurance of potential organic growth. Please submit photos with this report.
    Other Specify other issues in the comment box.

     

  • All concerns or questions regarding resident's bedroom furniture and bed configurations should be directed to the Student Housing Office. 

    If there is a problem with your bed set-up, or you would like to change your configuration, please login to MyLMU and go to the Student Housing Online Services Portal to submit your request.

  • Screens/Blinds

    While Facilities Management will create the work order on behalf of the Student Housing Office, their team will handle the repair.

    Any follow up regarding a screen or blind issue can be made with the Student Housing Office housing@lmu.edu or 310-338-2963.

  • Lockshop Services:

    If you are locked out of your room, contact One-Card Office, Public Safety, or Housing Area Offices who can provide you with a temporary card. DO NOT SUBMIT A WORK ORDER REQUEST. 

    If the lights are solid green and flashing red, the door battery needs replacement. Please call us or submit work order for immediate assistance.  

    If the lights alternate between red and green, this is an encoding issue. Please visit Area Office/ Housing Office to reencode card. DO NOT SUBMIT A WORK ORDER REQUEST. 

  • Pest Control Instructions:

    Please read the information below carefully:

    You are reporting your room is in need of attention with regard to EcoLab Pest Elimination.

    In order to proceed with treatment of your living space, we require a release to be signed by all students in the living space.  Please submit this release form, by clicking here.

    Or you can log on to your MyLMU -> Go to System Log Ins -> Student Housing Online Services -> My Housing -> Forms and complete the EcoLab Pest Elimination Form. Upon receipt of all remaining forms, EcoLab will be dispatched to your space and treatment will take place. 

    By clicking the submit button below, I understand a work order will be generated for my request, however, additional action is required of me.

  • Potential Organic Growth Instructions:

    FM and EHS will be scheduling a time to assess the potential organic growth in your unit. The Resident Director of your building will be reaching out to all residents in the space via email to confirm the date and time of assessment. Please note that all windows in the unit must be closed at least 24hrs before the assessment.
